A. For NEW Application

  1. Filled out Application Form
  2. Barangay Clearance (where Business Establishment is located)
  3. Sanitary/Health Permit from RHU
  4. Fire Clearance Certificate from Bureau of Fire Protection
  5. Zoning Clearance from Municipal Planning & Development Office (MPDO)
  6. Community Tax Certificate (Single/Corporation)
  7. SEC/DTI Registration
  8. Office location sketch
  9. Picture 1pc (2X2) of Proprietor (Single) or President/Treasurer (Corporation)
  10. Market Clearance for establishments located in Balamban Public Market
  11. Governor’s Permit from Cebu Provincial Capitol (For Sand & Gravel, Hollow Blocks Making, Cockpit Operation, Resorts, Printing Shops with 1Million & above capitalization)
  12. Philippine Contractors Accreditation Board (PCAB) License (For Contractors, Subcontractors and Specialty Contractors)
  13. Municipal Environment & Natural Resources Office (MENRO) Clearance certificate on SOLID WASTE MANAGEMENT SEMINAR/ECC
  14. Clearance on Calibration of Weights & Measures from the Treasury Office
  15. Certificate on Annual Orientation Seminar on STD for all videoke/resto bars from the Municipal Health Office
  16. Building/Occupancy Permit and Locational Clearnace from Mun. Engineering’s Office.

B. For RENEWAL Application

  1. Filled out Application Form
  2. Barangay Clearance (where Business Establishment is located)
  3. Sanitary/Health Permit from RHU
  4. Fire Clearance Certificate from Bureau of Fire Protection
  5. Community Tax Certificate (Single/Corporation)
  6. SSS/Philhealth/Pag-Ibig/BIR Clearance
  7. Market Clearance for establishments located in Balamban Public Market
  8. Governor’s Permit from Cebu Provincial Capitol (For Sand & Gravel, Hollow Blocks Making, Cockpit Operation, Resorts, Printing Shops with 1Million & above capitalization)
  9. Municipal Environment & Natural Resources Office (MENRO) Clearance certificate on SOLID WASTE MANAGEMENT SEMINAR/ECC
